Service bulletins for the 2016 Mazda CX-5

A bulletin is a manufacturer's communication to its dealers, as NHTSA indexes it. The summary is NHTSA's. The document is the manufacturer's: read it at NHTSA. A bulletin is not a recall, and listing a model year does not mean every vehicle of it is concerned.

It does not mean there is an actual FSC defect. In some cases the customer may have a concern with the Lane Departure Warning System (LDWS) and/or High Beam Control system (HBC). In other cases this DTC might be found without a related customer concern.
09-014-16-3132a2021-01-28. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS:CONCEALMENT DEVICES; EXTERIOR LIGHTING. Some vehicles equipped with LED or Discharge type headlights may exhibit white milky stains on the inner sur-face of the headlight lens. Some customers may complain of the poor appearance. This concern is caused by gas that emits from a gasket in the lamp unit. The gas adheres to the inner surface of the headlight lens and reacts with the anti-clouding coating and moisture in the air to create white milky stains. The gasket material has been improved to eliminate this concern.
09-010-19-36242019-08-15. Service bulletin. VISIBILITY; SERVICE BRAKES, HYDRAULIC; EXTERIOR LIGHTING; ELECTRICAL SYSTEM. Some customers may experience one or more electrical malfunctions after a vehicle body repair. The newly painted fender stay (2) may be causing a poor front harness (1) ground connection. The front fender stay (2) newly painted surface at ground point (3), where attachment bolt (4) is seated, may create a poor front harness ground connection. Customers concern can be resolved by sanding off paint at ground point (3) where attachment bolt (4) is seated. After installing the bolt (4), apply touch-up paint to the exposed base material area of the front fender stay.
09-003-19-3555a2019-03-20. Service bulletin. EXTERIOR LIGHTING; EXTERIOR LIGHTING:HEADLIGHTS; EXTERIOR LIGHTING:TAIL LIGHTS. Some customers may complain of condensation/fog inside the front (headlamp/headlight) or rear (taillamp/taillight) combination lights. The recent new models have been fitted with a clear lens which can cause the symptom to be more noticeable, however, it does not affect the performance of the light. This condensation/fog is a natural phenomenon that occurs when there is a fairly large temperature difference between the inside and the outside of the light housing. It is normal for very small water drops to appear in certain locations on the light units where the air is stagnant (condensation), making the lens look whitish (fog). This happens mostly in the corners and narrow spaces, and will typically clear during warmer ambient temperatures, or when the lights have been on for some time.

Some customers may notice that the Lane Departure Warning System (LDWS) and High Beam Control (HBC) (amber) warning lights are on when they start driving around dusk or dawn under low temperatures. At that time, the windshield is clear but the lens of the Forward Sensing Camera (FSC) is foggy. The FSC may not function properly under the following conditions: ? Foggy windshield ? Foggy FSC lens ? Heavy rain, dense fog, snow and direct sun light ? No or poor lane markings. When the FSC control system detects the foggy lens of the camera, the system deactivates LDWS and HBC functions.

09-001-17-3296b2017-10-04. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS; STRUCTURE; ELECTRICAL SYSTEM. When dealing with customer complaints of excessive condensation and/or water within the headlamp unit, first determine if all the bulb sockets (including O-ring), seals and covers are present and correctly installed. In most cases you will find that the Bulb Seal No. 1 (A) or HID Bulb Seal No. 1 (B) and Bulb Seal No. 2 (C) socket covers have not been correctly installed or are missing due to improper prior repairs (i.e., customer replacing the headlamp bulbs or body shop not transferring them when replacing the headlamp unit(s) due to a collision). Any damage caused by missing socket covers, HID module seal (D), or missing bumper brackets (E) is not covered by warranty policy.
150031732842017-09-27. Service bulletin. EXTERIOR LIGHTING:FOG LIGHTS; EQUIPMENT:OTHER:OWNERS/SERVICE/OTHER MANUAL. ? After installing the new Forward Sensing Camera (FSC), frequency of the disabled function with the LDWS/HBC warning lights ON due to foggy FSC lens will be reduced. However, the warning message will be displayed on the Multi-Information Display (MID) or the center display when FSC lens is foggy. ? The foggy FSC lens may occur at some weather conditions and is not caused by a defect of the parts. When the warning message is displayed, drive the vehicle for 10 minutes or more with the defroster ON to defogthe FSC lens.
SA-057-172017-08-10. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS. High Beam Control (HBC) system frequently asked questions (FAQ)
09-008-17-33242017-08-10. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS. Take caution when cleaning plastic headlight lenses. Damage can occur when using some cleaners that contain strong acid, alkaline cleanser or organic solvent. Damage caused by using these cleaners is not covered by Mazda's New Vehicle Limited Warranty term.
15-001-16-31262017-06-09. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS; VISIBILITY:WINDSHIELD; EQUIPMENT:OTHER:OWNERS/SERVICE/OTHER MANUAL. SOME VEHICLES MAY EXPERIENCE ONE OF THE FOLLOWING 2 SYMPTOMS, EVEN THOUGH THE WINDSHIELD IS CLEAR: 1. LANE DEPARTURE WARNING SYSTEM (LDWS) WARNING LIGHT TURNS ON AT IDLE. DTC C1001:92 (POOR FUNCTIONING OF FORWARD SENSING CAMERA (FSC)) IS STORED IN MEMORY.2.ADAPTIVE LED HEADLIGHTS (ALH) WARNING LIGHT TURNS ON WHILE DRIVING. DTC C1001:92 (POOR FUNCTIONING OF FORWARD SENSING CAMERA (FSC)) IS STORED IN MEMORY.UNDER CERTAIN DRIVING CONDITIONS, THE FORWARD SENSING CAMERA (FSC) MAY DETECT A DIRTY WINDSHIELD (NOT CLEAR).
09-014-16-3132A2017-05-30. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS.
09-003-16-31302017-05-26. Service bulletin. EXTERIOR LIGHTING:TAIL LIGHTS; EXTERIOR LIGHTING:HEADLIGHTS. SOME CUSTOMERS MAY COMPLAIN OF CONDENSATION/FOG INSIDE THE FRONT (HEADLAMP/HEADLIGHT) OR REAR (TAILLAMP/TAIL-LIGHT) COMBINATION LIGHTS. THE RECENT NEW MODELS HAVE BEEN FITTED WITH A CLEAR LENS WHICH CAN CAUSE THE SYMP-TOM TO BE MORE NOTICEABLE, HOWEVER, IT DOES NOT AFFECT THE PERFORMANCE OF THE LIGHT. EXPLAIN TO THE CUSTOMER THAT THIS IS A NATURAL PHENOMENON AND NOT A QUALITY DEFECT. DO NOT ATTEMPT REPAIRS AS THIS IS CONSIDERED NORMAL, AND IS NOT COVERED UNDER WARRANTY. REPLACING THE LIGHT UNIT WILL NOT CORRECT THIS SYMPTOM.
09-049-14-29802017-05-11. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS. WHEN DEALING WITH CUSTOMER COMPLAINTS OF EXCESSIVE CONDENSATION AND/OR WATER WITHIN THE HEADLAMP UNIT, FIRST DETERMINE IF ALL THE BULB SOCKETS ARE PRESENT AND CORRECTLY INSTALLED.
1066M2017-02-22. Service bulletin. EXTERIOR LIGHTING. SOME CUSTOMERS MAY COMPLAIN ABOUT IMPROPER OPERATION OF THE AUTO-DIMMING MIRROR. THIS MAY BE CAUSED BY AN OBSTRUCTION OF THE FRONT AND/OR REAR LIGHT SENSOR(S) (E.G. HANGING A PARKING PERMIT FROM THE MIRROR). USE A SOFT CLOTH WITH MILD DETERGENT TO CLEAN THE MIRROR. HARSH DETERGENT MAY DAMAGE THE FRONT AND/OR REAR LIGHT SENSOR(S).
09-014/162016-07-14. Service bulletin. EXTERIOR LIGHTING:HEADLIGHTS; EQUIPMENT. WHITE MILKY STAINS ON INNER SURFACE OF HEADLIGHT LENS. SOME VEHICLES EQUIPPED WITH LED OR DISCHARGE TYPE HEADLIGHTS MAY EXHIBIT WHITE MILKY STAINS ON THE INNER SURFACE OF HEADLIGHT LENS. SOME CUSTOMERS MAY COMPLAIN OF POOR APPEARANCE. THIS CONCERN IS CAUSED BY GAS THAT EMITS FROM A GASKET IN THE LAMP UNIT. THE GAS ADHERES TO INNER SURFACEOF HEADLIGHT LENS AND REACTS WITH ANTI-CLOUDING COATING AND MOISTURE IN THE AIR TO CREATE WHITE MILKY STAINS. THE GASKET MATERIAL HAS BEEN IMPROVED TO ELIMINATE THIS CONCERN. CUSTOMERS HAVING THIS CONCERN SHOULD HAVE THEIR VEHICLE REPAIRED USING THE FOLLOWING REPAIR PROCEDURE.
