Service bulletin 09-005-26

A bulletin is a manufacturer's communication to its dealers, as NHTSA indexes it. The summary is NHTSA's. The document is the manufacturer's: read it at NHTSA. A bulletin is not a recall, and listing a model year does not mean every vehicle of it is concerned.

NHTSA's summary

If the right-side Blind Spot Monitoring (BSM) control module is replaced for any reason, it is also necessary to replace the left-side BSM control module in order to complete the radar aiming procedure. If the right-side BSM control module is replaced alone, "Automatic Configuration" and "BSM Radar Aiming" cannot be completed due to a programming error in the module. In most cases, if the left-side BSM control module is replaced alone, "Automatic Configuration" and "BSM Radar Aiming" can be completed without any problem (See CAUTION below for more details). Other possible symptoms that may be found if the right-side BSM module is replaced alone: BSM aiming fails “BSM OFF" indicator ON Center Display warning message BSM failsafe (No operation) DTC U3000:54 stored in memory DTC U1A4B:54 stored in memory (CX-3, MX-5 only) NOTE: Since a new BSM control module does not retain the vehicle specification information, it is necessary to acquire and store the vehicle specification information transmitted via CAN communication from the combination meter through automatic configuration after replacement.

If the right-side BSM control module is replaced alone, "Automatic Configuration" and "BSM Radar Aiming" cannot be completed due to a programming error in the module. 15-004-18-35392018-12-17. Service bulletin. Some customers may experience a Blind Spot Monitoring (BSM) system that does not detect a vehicle in the BSM alert area. The BSM indicator light on the outer door mirror glass will not illuminate and no DTC's are stored. The BSM system may operate when switching the ignition switch OFF, then ON. This may be caused by low battery voltage that stops its operation during the drive cycle, even if the voltage recovers. To eliminate this concern, the BSM control module logic has been modified to not to stop its operation when the voltage recovers.
09-031-18-34892018-09-10. Service bulletin. If it becomes necessary to replace the right-side Blind Spot Monitoring (BSM) control module, the configuration may fail even though the Workshop Manual procedure was performed properly. This may occur when the existing BSM control module has a hand written suffix on the label. Example: Applicable BSM control module part number DB2R-67-Y80B, where the suffix ("B") is handwritten on the module label: This concern is caused by the wrong suffix being written on the label with an older software version. To eliminate this concern, the production process has been changed.
15-005-17-33582017-10-04. Service bulletin. Some customers may complain that when passing a vehicle slowly, even if the vehicle is still in the alert area, the BSM indicator light on the outer mirror glass does not illuminate. Customers who have owned previous Mazda vehicles are more likely to complain. The BSM control module has been modified to address this customer concern.
