NHTSA's summary
Findings have concluded that there were some plunger assemblies installed out of tolerance. The gasket could chaff or lodge in the valve housing set if not corrected. Mounting flange gaskets will be sent to the customer so the discharge valve can be opened to inspect the plunger valve.
